IP Address and Cookies

Information Provided at Registration

MedConnect pledges to protect the privacy of all of our registered users. The MedConnect registration form requires users to give us contact information (name and e-mail address), and demographic information (ZIP code or age). To use some site functions users must volunteer personal health information (such as, height, weight, and past medical history). There is no financial information collected.

We use contact information from the registration form for internal purposes, to send the user news, information about new MedConnect site features, services or updates, information about our company, and promotional material or offers from some of our corporate partners. You will be given the opportunity to decline these services at the point of registration, and you may opt-out of receiving future mailings (see the Choice/Opt-out section following).

Demographic and personal health information is used to tailor the visitor's experience at our site, showing them content that we think they might be interested in, and displaying the content according to their preferences. Your health information is password-protected to prevent access by other users.

MedConnect will not sell or forward to third parties any individually identifiable personal information, e.g., your e-mail address, name, etc., that you supply during the registration process; this includes health insurers. You can be assured that our content providers will not have access to your personal information. MedConnect may, however, provide your personal health information and that of other registered users in aggregate form to third parties, but never in such a way that it violates your privacy. This means that we will never provide anyone's individual health records to any third party, although we may provide advertisers with information about the prevalence of certain health conditions in the overall group of MedConnect subscribers, and similar aggregate information.

Though we make every effort to preserve user privacy, we may need to disclose personal information when required by law wherein we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order or legal process served on our Web site.

MedConnect does not knowingly collect personal information from any person under the age of 13. We are committed to protecting the privacy of children, and this website is not designed or intended for children under the age of 13.

All personal, identifiable information is retained by Medical Network, Inc. (d.b.a. MedConnect.com) for a period of 5 years/indefinitely unless a member requests that the information be deleted. Online Surveys

We may run surveys on our site in which we ask visitors for contact information (such as their e-mail address). We use contact data from our surveys to send users information about our company. You have the option of choosing not to receive such e-mails at the time you register or at any time. (See Choice/Opt-Out section following.)

Public Forums

MedConnect makes chat rooms, forums, message boards, and/or news groups available to our users. Please remember that any information that is disclosed in these areas becomes public information, and you should exercise caution when considering the disclosure of your personal information. MedConnect is not responsible for material posted.

Security

This site has security measures in place to protect the loss, misuse, and alteration of the information under our control. Information provided to the various sections of the MedConnect site is password-protected, encrypted and protected by means of a firewall.

HIPAA

MedConnect is in full compliance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Choice/Opt-Out

MedConnect provides users the opportunity to opt-out of receiving communications from our partners and us.

When you complete registration or when you make changes to your profile (Edit Profile), you will be given the choice of receiving informational/promotional e-mails from MedConnect. You may opt-out of receiving e-mails on behalf of MedConnect at registration by removing the automatic check mark from the preference box for receiving e-mails and newsletters. You may also opt-out of receiving these e-mails and newsletters by changing your preferences in Edit Profile. You may remove all of the information you've stored with us through the "Edit Profile" screen. If you wish to remove your information, the process takes three business days and you will be notified via e-mail upon completion of the removal process.

If you wish to unsubscribe to the MedConnect newsletter, you also can do so within any of the newsletters you receive, or you can unsubscribe under "Contact Us" on our homepage.

Correct/Update/Remove

In addition, this site gives users the following options for opting out of receiving information from us, updating and removing personal information they have provided, and changing preferences with respect to receiving future communications:

If you choose to remove or not supply us with personal information you will be unable to access some of MedConnect.com's features. You will be unable to access personalized features such as trackers, E-Health Dashboard, Health Managers, Personal Health Record, personalized and dynamic content, and e-newsletters. You will still have access to our static content, risk assessments and un-personalized tools.

Notification of Changes

If we are going to use your personally-identifiable information in a manner different from that stated at the time of collection, we will notify you via e-mail. You will have a choice as to whether we use your information in this different manner. In addition, if we make any material changes in our privacy practices that do not affect user information already stored in our database, we will post a prominent notice on our Web site notifying users of the change. In some cases where we post the notice, we will also e-mail users who have opted to receive communications from us, notifying them of the changes in our privacy practices.
